Outpatient Services
Our vision is for people of all ages living in West Hertfordshire to be healthier and have better care that is joined-up and responsive to their individual needs, closer to where they live.
Through our engagement with the public we’ve heard about how important it is to ensure more people can access the care and support they need in their own community, rather than travel to hospital unnecessarily.
Several health services are now being delivered out in the community, and you can read more about these below.
Community Vasectomy Service
Herts Valleys CCG have commissioned Tollgate Clinic Ltd to deliver a community vasectomy service. This means that specially trained GPs will carry out procedures in enhanced GP premises.
Your GP refers you to this service and it is available to all west Herts patients following an initial consultation.
Hertfordshire community diabetes service
We have a local diabetes care pathway led by West Herts Hospitals NHS Trust in conjunction with other providers, and provide a range of services to support patients with diabetes including:
- National Pre-Diabetes Prevention Programme referral form and PIL on DXS
- Localised Diabetes Care Pathways
- HCT Diabetic Eye Screening
- HPFT Integrated Care Diabetes Referral Form
- Diabetic Foot
This service is available through referral from your GP.
Enhanced Community Respiratory Service
Many respiratory conditions can be treated at community locations. These include Obstructive Sleep Apnoea Referral Form, Community Respiratory Clinic Referral Form, Home Oxygen Referral Form, Pulmonary Rehabilitation.
Minor Surgery
Several GP practices across Herts Valleys CCG provide Minor Surgery for patients including:
- Incisions and Excision
- Joint Injections
This service is available through referral from you GP.
Palliative and End of Life Care Services
Patients can be referred by their GP for palliative care services in the community delivered by Peace Hospice Care, The Hospice of St Francis, Rennie Grove Hospice Care and Hertfordshire Community Trust depending on their location.

Marlowes Health and Wellbeing Centre
This is a newly-refurbished state-of-the-art health and wellbeing centre in the middle of Hemel Hempstead.
The centre brings together community mental and physical health services for adults and children provided by Hertfordshire Community NHS Trust (HCT) and Hertfordshire Partnership NHS Foundation Trust (HPFT).
Having these services together under one roof makes it more convenient for patients, service users, carers and their families. It also means that healthcare professionals are able to share expertise with each other to improve the treatment patients receive.
Two week wait GP urgent referral
A GP will make a two week wait GP urgent referral appointment to a hospital specialist if they are concerned that a patient may have cancer.
Out of 10 people referred to hospital in this way, 9 of these people DO NOT have cancer. However, for the small number of people who do, seeing a specialist quickly can mean the cancer is diagnosed early and is more likely to be treated successfully.
