Support for long term conditions
Long term conditions are defined as 'those chronic health conditions that cannot, at present, be cured'.
About 140,000 people in West Hertfordshire have one or more long term conditions such as diabetes, heart or lung disease. Long term conditions are a priority in our strategy, Your Care, Your Future (page 19, item 2.4.3).
Over 10% of people with long term conditions in west Hertfordshire feel they don't get enough support from local services so we have put together this list of links to local and national support services to make it easier for you to get support from.
