New adult integrated community health services contract

Herts Valleys CCG have announced the provision of new adult integrated community health services for people across west Hertfordshire. We have been working for some time on a major review of adult community health services and can today (29 January 2019) announce that the new contract will start on 1 October 2019.

The new specification for the services was developed in partnership with people who use and provide services currently and it is designed to turn the aspirations of our Your care, your future programme into reality. The new approach also puts into practice the national expectations around fully joined-up services that are set out in NHS England’s Long Term Plan. We know that the many thousands of patients who use the service will notice a real transformation in their care.

This new way of working will include bringing physical and mental health, primary, social and voluntary care organisations closer together so that patients get the benefits of co-ordinated care. For example, patients with complex needs will have a designated ‘key worker’ to really make sure we are delivering care that is co-ordinated. This will be real on-the-ground neighbourhood-based integration.

The focus will also be on preventing people from becoming ill and on intervening sooner so that we can avoid people getting sicker. Some of that will be about providing more support to individual patients to help them look after themselves better – to keep healthy and avoid needing hospital care.

In doing this, we will direct resources to where they are most needed and will be more targeted in a way that will help us reduce the inequalities in health that are still evident in our patch. All of this has been our plan for some time – the new model will actually deliver it.

The provider of this major new contract and model of care will be Central London Community Healthcare Trust (CLCH) – a well-established NHS organisation and already familiar with west Hertfordshire, as providers of integrated community respiratory care. During this extensive process, CLCH evidenced their strong track record in delivering transformation and demonstrated their ability to respond effectively to our requirements.

The current provider, Hertfordshire Community Trust (HCT), will continue to deliver a wide range of services in the county, including in this area. The change in provider of adult community services – delivering a new model of care – is not a reflection of the current quality of service and care that HCT provides and we retain a very high level of confidence in our colleagues in HCT.

We want people to know that those staff currently providing adult community services to patients here, and employed by HCT, will be moving to the new provider when the contract goes live. This means that patients here in west Hertfordshire will still be looked after by the highly skilled and caring staff currently employed by HCT.
