This includes redevelopment projects – such as the development of health and wellbeing hubs to provide more care closer to home – and work to deliver new models of care for certain services.

Improving services today

Work has already started to improve health and social care services in Dacorum.

A Rapid Response service to support older people is in the process of being established in Dacorum. This will involve a team of health, social care and mental health professionals joining up to respond speedily in crisis situations to avoid older people being admitted to hospital or help them be discharged more quickly.

New Hemel Hempstead Hospital (Health and wellbeing hub)

Local clinicians, patients and community representatives are working on plans for a new Hemel Hempstead Hospital that will be the 'health and wellbeing hub' Healthcare in communities for patients in Hemel Hempstead and the surrounding area.

The £30million facility will provide a range of outpatient services including diagnostic tests and treatments such as pathology, x-rays, ultra sound scans, minor surgery, physiotherapy and rehabilitation as well as urgent care services.

The new hospital will be developed on the existing hospital site will not need as much space. This creates potential for developing the rest of the site, including building some new homes.

We are exploring the finances and are working towards producing a strategic outline case for the hub later this year. This work is being led by Herts Valleys Clinical Commissioning Group, Dacorum Borough Council and Hertfordshire County Council Public Health service.

New Health and Wellbeing Centre at The Marlowes

Hertfordshire Partnership Foundation NHS Trust and Hertfordshire Community NHS Trust are working to develop a new health and wellbeing centre in The Marlowes to serve their existing patients. A £6million project will see services delivered from a single location and patients benefiting from more joined-up mental and physical health services. The new centre is due to open in autumn 2017

Hertsmere

Improving services now

A Rapid Response service is supporting older people and adults across Hertsmere. This involves a team of health, social care and mental health professionals joining up to respond speedily in crisis situations to avoid older people being admitted to hospital or help them be discharged more quickly.

Borehamwood Health and wellbeing hub

Local clinicians and patients are working on plans for a Healthcare in communities in Elstree Way, Borehamwood. Careful planning and analysis is taking place to make sure the hub meets the needs of the local population. This will help build a strategic outline case for the Borehamwood hub, which is due to be published later this year.

Work to develop the Borehamwood health and wellbeing hub is being led by Herts Valleys Clinical Commissioning Group and Hertsmere Borough Council.

St Albans & Harpenden

Harpenden Health and wellbeing hub

Local clinicians and patients are currently working on plans for a health and wellbeing hub in Harpenden.

Hertfordshire Community NHS Trust is working with the community including St Albans City and District and town councillors, The Harpenden Society, Red House Forum and the St Albans and Harpenden Patient Group to discuss the redevelopment of the Harpenden Memorial site and ideas for the hub. They have a local resident on their project board and have a programme of future engagement activity.
